Job Summary: The Population Health Data Analyst develops and maintains reports, dashboards, patient panels, and data tools that support population health, clinical quality improvement, community health operations, and organizational decision-making. Reporting to the Director of Clinical Informatics, the Analyst collaborates with the Population Health Manager and clinical, operational, and quality leaders to turn data into reliable, usable information.
