Description Lexington Country Place is seeking an MDS Coordinator to join the team! The MDS Coordinator reports directly to the Director of Nursing. Schedule: 8 hours per week PURPOSE The MDS Coordinator is responsible for Overall case management of the facility Medicare/Managed Care Program.
