Position Summary: Community Care Plan (CCP) is proud to offer an internship program that will prepare students for the workforce. This is a great internship opportunity to get professional hands-on experience in working with the Population Health and Concierge Care Coordination Department. The Intern will directly assist the department leadership with a wide range of projects/ tasks related to healthcare data analysis, reporting, performance measurement, and population health/care management operations.
